American culture needs a new social contract to deal with advancing illness, frailty, and death itself. As a country, we spend enormous resources to extend life, but with little regard for the quality of life. Can we do a better job of aligning medical resources with personal values, preferences and desires to achieve a balance between the quantity and quality of life?
