At Trillium Institute, we are committed to helping people maintain the quality of their lives by making intentional choices about medical care based on personal values, preferences and desires. It is important for everyone to document their choices through Advance Care Planning, and to share those documents with family, friends, and the medical community. We partner with Making Choices Michigan to help you articulate and communicate your values, and modify them as your needs and preferences change over time.
We also offer Navigation Services to help selected patients and their families deal with the daunting challenges of discerning treatment options consistent with patients’ values and priorities, coordinating care between multiple medical care resources, and offering compassionate and mindful emotional support for medical decisions.
